Application Requirements
Grant Requests must be in writing and should be addressed to:

Grant requests may be submitted at any time. However, the Board of Directors of the Foundation must approve all grants prior to disbursement. The Directors meet several times annually in order to review requests for grants.
Grant requests must include the following:
- Full name of the organization;
Mailing address;
Contact person/title;
Telephone number;
Type of Organization, i.e. corporation, foundation, association, etc.;
A summary of the organization's goals and a description of the services provided by the organization;
Length of time in operation; and
A list of all directors and officers.
- A copy of a letter from the Internal Revenue Service confirming that the organization is exempt from federal income tax.
- A description of the project or purpose for which funds are being requested.
- A budget for the project showing the amount and source of all support for the project and a detailed listing of expenses.
- A copy of the requesting organization's most recent financial statement.
- A statement that the requested funds will be used by the organization in Central or Eastern Pennsylvania.
- A request for a specific dollar amount and the date when funds are needed.
- Any additional information that might be helpful to the Directors in evaluating the request.
